Planning Your First Visit

Initial Consultation with Your Physician


Planning your first visit to HRC.The purpose of your initial consultation at HRC Fertility (HRC) is to evaluate your medical history and discuss a personalized treatment plan for you. Our goal is to make you an informed participant in your treatment and to provide expert reproductive care in a compassionate setting. Please plan on staying one to two hours with us, including waiting time, as you will be spending quality time with both your physician and nurse coordinator.

Because we feel it is extremely important to obtain as much information as possible, we encourage and invite both partners to attend the initial consultation. In your first consultation, you will have one-on-one time with your doctor, followed by a baseline ultrasound, and have one-on-one time with your nurse coordinator.

To better prepare for your initial visit at HRC, below are some common questions you may have prior to your appointment.

What should I bring to the initial visit?

In order to assist your doctor, it is preferred that you submit medical records or lab results in advance so there is adequate time to review your medical history in detail.  If needed, we can also provide you with a medical records request. 

HRC will mail you a welcome packet to bring with you on the day of your consultation.  We include a patient questionnaire that you will need to have completed prior to your appointment.  You can also prepare questions for your doctor prior to your appointment using the You and Your Doctor form.

Will I meet with a Financial Advisor?

Prior to initiating treatment, a financial consultation will need to be scheduled. This meeting is designed to provide you with information regarding HRC’s financial programs, and if applicable, to understand your insurance coverage. Learn more about our financial programs.

Do you take Insurance?

In order to verify your insurance benefits, please provide us with a fax copy of your insurance card or you can provide the information over the phone.  We are currently contracted with most PPO plans and select HMO groups.  If you have an HMO, an authorization must be submitted prior to your appointment.  We have also created a downloadable Insurance Question form to help you when gathering information from your insurance carrier.

What are the fees for your services and do you have any payment plans available?

We have a price sheet for patients to view online.  When you meet with your financial advisor, he or she will discuss with you the financial packages associated with your treatment plan. We have financial plans that meet almost every budget.

Are you available on the weekends?

We are open every day, but we are open on the weekends for routine appointments and procedures.

Prior to Cycle Start

Prior to cycle start, all necessary lab screening/diagnostic testing must be complete, consents must be signed, fees paid, insurance benefits verified, and (if participating in a Low Cost Two-Cycle* - or Three-Cycle* or our Refund Guarantee Program*) qualification and approval must be met.

* Please note that qualification for these programs is subject to approval. Any multiple cycle plans involving egg donors must be of 34 years of age or less.
